Penyakit Kanker adalah suatu penyakit yang disebabkan oleh pertumbuhan sel-sel jaringan tubuh yang tidak normal. Sel-sel kanker akan berkembang dengan cepat, tidak terkendali, dan akan terus membelah diri, selanjutnya menyusup ke jaringan sekitarnya (invasive) dan terus menyebar melalui jaringan ikat, darah, dan menyerang organ-organ penting serta syaraf tulang belakang. Dalam keadaan normal, sel hanya akan membelah diri jika ada penggantian sel-sel yang telah mati dan rusak. Sebaliknya sel kanker akan membelah terus meskipun tubuh tidak memerlukannya, sehingga akan terjadi penumpukan sel baru yang disebut tumor ganas. Penumpukan sel tersebut mendesak dan merusak jaringan normal, sehingga mengganggu organ yang ditempatinya. Kanker dapat terjadi diberbagai jaringan dalam berbagai organ di setiap tubuh, mulai dari kaki sampai kepala. Bila kanker terjadi di bagian permukaan tubuh, akan mudah diketahui dan diobati. Namun bila terjadi didalam tubuh, kanker itu akan sulit diketahui dan kadang - kadang tidak memiliki gejala. Kalaupun timbul gejala, biasanya sudah stadium lanjut sehingga sulit diobati.

Perbedaan Tumor dan Kanker

Tumor ada dua macam yaitu tumor jinak dan tumor ganas. Tumor jinak hanya tumbuh dan membesar, tidak terlalu berbahaya, dan tidak menyebar ke luar jaringan. Sedangkan tumor ganas adalah kanker yang tumbuh dengan cepat dan tidak terkendali dan merusak jaringan lainnya.

Jenis - jenis kanker yaitu :

- Karsinoma
Yaitu jenis kanker yang berasal dari sel yang melapisi permukaan tubuh atau permukaan saluran tubuh, misalnya jaringan seperti sel kulit, testis, ovarium, kelenjar mucus, sel melanin, payudara, leher rahim, kolon, rectum, lambung, pancreas, dan esofagus.

- Limfoma
Yaitu jenis kanker yang berasal dari jaringan yang membentuk darah, misalnya jaringan limfe, lacteal, limfa, berbagai kelenjar limfe, timus, dan sumsum tulang. Limfoma spesifik antara lain adalah penyakit Hodgkin (kanker kelenjar limfe dan limfa)

- Leukemia
Kanker jenis ini tidak membentuk massa tumor, tetapi memenuhi pembuluh darah dan mengganggu fungsi sel darah normal.

- Sarkoma
Yaitu jenis kanker dimana jaringan penunjang yang berada dipermukaan tubuh seperti jaringan ikat, termasuk sel - sel yang ditemukan diotot dan tulang.

- Glioma
Yaitu kanker susunan syaraf, misalnya sel-sel glia (jaringan penunjang) di susunan saraf pusat.

- Karsinoma in situ
Yaitu istilah yang digunakan untuk menjelaskan sel epitel abnormal yang masih terbatas di daerah tertentu sehingga masih dianggap lesi prainvasif (kelainan/luka yang belum menyebar)

KANKER SERVIKS ATAU KANKER LEHER RAHIM

Mari kita membahas mengenai penyebab dan gejala kanker leher rahim, tapi sebelumnya kita perlu tahu apa sich kanker leher rahim tersebut? “Kanker leher rahim” adalah tumor ganas yang tumbuh di daerah leher rahim (serviks), yaitu suatu daerah pada organ reproduksi wanita yang merupakan pintu masuk ke arah rahim yang terletak antara rahim (uterus) dan liang senggama (vagina).

Kanker leher rahim terjadi jika sel-sel yang ada di daerah tersebut membelah secara tak terkendali dan menjadi abnormal. Jika sel-sel tersebut terus membelah, maka akan terbentuk suatu massa jaringan yang disebut tumor. “Tumor” dapat bersifat jinak atau ganas. Jika tumor tersebut menjadi ganas, maka keadaanya disebut sebagai kanker leher rahim.

 Penyebab kanker leher rahim

Penyebab dari terjadinya kelainan pada sel-sel leher rahim tersebut tidak diketahui secara pasti, tetapi terdapat beberapa faktor resiko yang dapat berpengaruh terhadap terjadinya kanker serviks tersebut :

 1. HPV (Human Papilloma Virus)
          HPV adalah suatu virus yang dapat menyebabkan terjadinya kutil pada daerah genital (kondiloma akuminata), yang ditularkan melalui hubungan seksual. HPV sering diduga sebagai penyebab terjadinya perubahan yang abnormal dari sel-sel leher rahim.

 2. Merokok
           Tembakau dapat menurunkan sistem kekebalan tubuh dan mempengaruhi kemampuan tubuh untuk melawan infeksi HPV pada serviks.

 3. Hubungan seksual pertama dilakukan pada usia dini

 4. Berganti-ganti pasangan seksual

 5. Gangguan sistem kekebalan tubuh

 6. Pemakaian pil KB

 7. Infeksi herpes genitalis atau infeksi klamidia menahun

 8. Penggunaan pembalut yg kualitasnya rendah

 9. penggunaan bahan kimia berbahaya yg terlalu berlebihan untuk rahim/vagina

 10. acuh/cuek terhadap masalah2 miss v yg sudah berlebihan dan tdk ditangani

      contohnya keputihan yg berlebihan, gatal2, iritasi, dan sebagainya

 Gejala Kanker Leher Rahim

Pada awalnya perjalanan penyakit dari kanker leher rahim dapat berupa pembakal kanker atau lesi prakanker. Perubahan prekanker ini biasanya tidak menimbulkan gejala dan tidak terdeteksi kecuali jika wanita tersebut menjalani pemeriksaan panggul atau pap smear.

          Gejala biasanya baru muncul ketika sel serviks yang abnormal berubah menjadi keganasan dan menyusup ke jaringan sekitarnya. Pada saat ini dapat timbul gejala seperti gangguan menstruasi, perdarahan vagina, serta keputihan.

 Pendarahan vagina yang tidak normal seperti :

• Pendarahan di antara periode menstruasi yang regular.
• Periode menstruasi yang lebih lama dan lebih banyak dari biasanya.
• Pendarahan setelah hubungan seksual atau pemeriksaan panggul.
• Rasa sakit saat berhubungan seksual.
• Timbulnya keputihan yang bercampur darah dan berbau.
• Nyeri panggul dan gangguan atau bahkan tidak bisa buang air kecil.

 Bila mengalami salah satu gejala di atas, segeralah hubungi dokter! Kondisi di atas tidak selalu disebabkan oleh kanker serviks, tapi dapat merupakan tanda infeksi vagina yang perlu segera diobati.

STADIUM

         Untuk tumbuh menjadi kanker leher rahim dibutuhkan beberapa tahun sejak sel-sel leher rahim mengalami perubahan. Sel-sel leher rahim abnormal yang bukan merupakan sel kanker namun dapat berkembang menjadi kanker disebut dengan cervical intra – epithelial neoplasia (CIN). CIN juga disebut sebagai sel-sel prekanker yang jika tidak ditangani lebih lanjut akan berpotensi untuk berkembang menjadi kanker. Namun tidak semua wanita yang memiliki CIN akan menderita kanker. Keberadaan CIN identik dengan displasia (Anonim, 2003c).

Perkembangan kanker servik meliputi displasia ringan (5 tahun), displasia sedang (3 tahun), displasia berat (1 tahun) sampai menjadi kanker stadium 0. Tahap pra kanker ini sering tidak menimbulkan gejala (92%), selanjutnya masuk tahap kanker invasif berupa kanker stadium I sampai stadium IV (Anonim, 2003).

Perkembangan kanker leher rahim dibagi menjadi 5 stadium berdasarkan ukuran tumor, kedalaman penetrasi pada leher rahim dan penyebaran kanker di dalam maupun diluar leher rahim. Stadium-stadium tersebut adalah sebagai berikut  :

1. Stadium 0 : Terjadi pertumbuhan kanker (karsinoma) pada jaringan epitel leher rahim 
2. Stadium I : Pertumbuhan kanker masih terbatas pada leher rahim
2.1. Ia :  Secara mikroskopis, kanker telah menginvasi jaringan (terjadi penetrasi). Ukuran invasi sel kanker : kedalaman < 5 mm, sedangkan lebarnya < 7 mm
2.1.1.  Ia1 : Ukuran invasi mempunyai kedalaman < 3 mm dan lebar < 7 mm
2.1.2.  Ia2 :  Kedalaman invasi > 3 mm dan < 5 mm, lebar < 7 mm
2.2. Ib :  Terjadi lesi yang ukurannya lebih besar dari lesi yang terjadi pada stadium Ia
2.2.1.  Ib1 : Ukuran tumor < 4 cm
2.2.2.  Ib2 : Tumor > 4 cm

 3. Stadium II :  Karsinoma meluas sampai keluar leher rahim tetapi belum sampai dinding pelvis; karsinoma menyerang vagina tapi belum mencapai 1/3 vagina bagian bawah.

  3.1. IIa : Belum ada parameter yang jelas.

  3.2. IIb : Parameter jelas.

4. Stadium III : Karsinoma meluas ke dinding pelvis; pada pemeriksaan rektal, tidak terlihat adanya ruang kosong antara tumor dan dinding pelvis; tumor menyerang 1/3 vagina bagian bawah; pada semua kasus juga ditemukan adanya hidronefrosis atau ginjal tidak berfungsi.

4.1. IIIa : Kanker tidak menjalar ke dinding pelvis, tapi menyerang 1/3 vagina bagian bawah
4.2. IIIb : Menjalar ke dinding pelvis, terjadi hidronefrosis atau kegagalan fungsi ginjal, atau keduanya

Examination for typhoid fever

                                                                   TEST WIDAL
          Pemeriksaan widal adalah salah satu pemeriksaan serologi yang bertujuan untuk menegakan diagnosa demam tipoid.Uji widal positif artinya ada zat anti (antibodi) terhadap kuman Salmonella, menunjukkan bahwa seseorang pernah kontak/terinfeksi dengan kuman Salmonella tipe tertentu. Pemeriksaan ini masih banyak dipakai di negara-negara berkembang dikarenakan biayanya yg relatif terjangkau dan hasilnyapun dapat diketahui dengan segera.

          Pemeriksaan widal bertujuan untuk mendeteksi adanya antibodi (kekebalan tubuh) terhadap kuman Salmonela dengan cara mengukur kadar aglutinasi antibodi terhadap antigen O dan H dalam sampel darah. Tubuh kita akan membentuk antibodi jika terpapar kuman Salmonela typhi, baik kuman yg masuk secara alamiah dan menyebabkan sakit, kuman yg masuk namun tidak menunjukan gejala (karier) ataupun melalui vaksinasi.
          Pada pasien yg saat ini tidak sedang sakitpun pemeriksaan widal mungkin saja menunjukan hasil yang positif, pada pasien yang mendapat vaksinasi tipoid hasil pemeriksaan widalnyapun bisa positif. Perlu diperhatikan sobat analis semua bahwa pemeriksaan widal yang positif bukan hanya terjadi pada infeksi kuman Salmonella typhi, namun juga akibat infeksi kuman Salmonella yang lain, sehingga pada saat ini pemeriksan ini tidak dapat lagi dijadikan acuan pemeriksaan yg spesifik terhadap penyakit tipoid.
          Pengambilan sampel pasien untuk pemeriksaan widal juga kadang kurang tepat waktunya, karena berdasarkan perjalanan penyakitnya antibodi terbentuk pada hari ke 5-7 ke atas, sehingga tidak bijak jika pemeriksaan widal dilakukan sebelum hari ke 5, dan kalaupun pada pemeriksaan wideal didapat hasil yg positif pada sebelum hari ke 5 maka yg terdeteksi tersebut dimungkinkan antibodi yg terbentuk tersebut berasal dari infeksi sebelumnya.Mengingat adanya kelemahan tersebut maka pada saat ini di era kemajuan teknik pemeriksaan laboratorium, pemeriksaan tersebut seharusnya tidak lagi menjadi pilihan, meskipun masih saja dilakukan di laboratorium-laboratorium pratama atau di daerah-daerah dimana teknik pameriksaan yg lain belum tersedia, namun tetap memperhatikan hal-hal penting dalam menegakan diagnosa tipoid yaitu tanda klinis yg menunjang (demam lebih dari 7 hari, anamnesis dan pemeriksaan fisik) atau dilakukan pemeriksaan widal serial pada minggu ke 1 dan minggu ke 2 dan pada periode convalescence saatdemam mulai turun (pemeriksaan cukup bermakna jika terdapat kenaikan titer 2-4 kali).
Pemeriksaan ini walaupun lebih baru namun pemeriksaannya kurang praktis dan harganya lebih mahal.
Pemeriksan yang tepat
       Dari gejala-gejala klinik yang mengarah ke arah demam tipoid, pemeriksaan di bawah ini seharusnya direkomendasikan oleh seorang dokter untuk menegakan diagnosanya :
1. Pemeriksaan laboratorium sederhana, sehingga pada pemeriksaan tersebut didapatkan leukopenia, aneusinofilia, trombositopenia, dan limpositosos relatif
2. Pemeriksaan IgM Anti Salmonella (Tubex TF)
Pada pemeriksaan ini dapat mendeteksi adanya antigen spesifik dari kuman salmonella walaupun pemeriksaan dilakukan dalam minggu pertama setelah demam.
3. Kultur darah
Pemeriksaan ini merupakan gold standar untuk pemeriksaan demam tipoid
4. PCR (polymerase Chain Reaction)
Beberapa hal yang sering disalahartikan :
1. Pemeriksaan widal positif dianggap ada kuman dalam tubuh, hal ini pengertian yang salah. Uji widal hanya menunjukkan adanya antibodi terhadap kuman Salmonella.
2. Pemeriksaan widal yang diulang setelah pengobatan dan menunjukkan hasil positif dianggap masih menderita tifus, ini juga pengertian yang salah.
Setelah seseorang menderita tifus dan mendapat pengobatan, hasil uji widal tetap positif untuk waktu yang lama sehingga uji widal tidak dapat digunakan sebagai acuan untuk menyatakan kesembuhan.
Hasil ulang pemeriksaan widal positif setelah mendapat pengobatan tifus, bukan indikasi untuk mengulang pengobatan bilamana tidak lagi didapatkan gejala yang sesuai.
Hasil uji negatif dianggap tidak menderita tifus :
Uji widal umumnya menunjukkan hasil positif 5 hari atau lebih setelah infeksi. Karena itu bila infeksi baru berlangsung beberapa hari, sering kali hasilnya masih negatif dan baru akan positif bilamana pemeriksaan diulang. Dengan demikian,hasil uji widal negatif,terutama pada beberapa hari pertama demam belum dapat menyingkirkan kemungkinan tifus.
Untuk menentukan seseorang menderita demam tifoid :
1.  Tetap harus didasarkan adanya gejala yang sesuai dengan penyakit tifus.
2. Uji widal hanya sebagai pemeriksaan yang menunjang diagnosis.
Seorang tanpa gejala,dgn uji widal positif tidak dapat dikatakan menderita tifus.

Memang terdapat kesulitan dalam interpretasi hasil uji widal karena kita tinggal di daerah endemik,yang mana sebagian besar populasi sehat juga pernah kontak atau terinfeksi, sehingga menunjukkan hasil uji widal positif. Hasil survei pada orang sehat di Jakarta pada 2006 menunjukkan hasil uji widal positif pada 78% populasi orang dewasa. Untuk itu perlu kecermatan dan kehatihatian dalam interpretasi hasil pemeriksaan widal.
PENILAIAN
Titer widal biasanya angka kelipatan :1/32 , 1/64 , 1/160 , 1/320 , 1/640.
- Peningkatan titer uji Widal 4 x (selama 2-3 minggu) : dinyatakan (+).
- Titer 1/160 : masih dilihat dulu dalam 1 minggu kedepan, apakah ada kenaikan titer. Jika
ada, maka dinyatakan (+).
- Jika 1 x pemeriksaan langsung 1/320 atau 1/640, langsung dinyatakan (+) pada pasien dengan gejala klinis khas.
Uji Widal didasarkan pada :
- Antigen O ( somatic / badan )
- Antigen H ( flagel/semacam ekor sebagai alat gerak )
Jika masuk ke dalam tubuh kita, maka timbul reaksi antigen-antibodi.ANTIBODI terhadap :
Antigen O : setelah 6 sampai 8 hari dari awal penyakit.
Antigen H : 10-12 hari dari awal penyakit.
Uji ini memiliki tingkat sensitivitas dan spesifitas sedang (moderate).
Pada kultur yang terbukti positif, uji Widal yang menunjukkan nilai negatif bisa mencapai 30 persen.
Beberapa keterbatasan uji Widal ini adalah:
1.Negatif Palsu
Pemberian antibiotika yang dilakukan sebelumnya (ini kejadian paling sering di negara kita, demam →di beri antibiotika →tidak sembuh dalam 5 hari →dilakukan test Widal) menghalangi respon antibodi.
Padahal sebenarnya bisa positif jika dilakukan kultur darah.
2. Positif Palsu
  Beberapa jenis serotipe Salmonella lainnya (misalnya S. paratyphi A, B, C) memiliki antigen O dan H juga, sehingga menimbulkan reaksi silang dengan jenis bakteri lainnya, dan bisa menimbulkan hasil positif palsu (false positive).
Padahal sebenarnya yang positif kuman non S. typhi (bukan tifoid).
  Beberapa penyakit lainnya : malaria, tetanus, sirosis, dll.

Bahaya Merokok

           Rokok adalah benda berbentuk silinder kecil yang dalamnya berisi tembakau yang telah di campur dengan bahan-bahan lain. Rokok dibakar pada ujungnya untuk kemudian dihisab asapnya dan dibuang lagi. Rokok sudah ada sejak jaman dulu, dan sekarang rokok sudah sangat marak. Kadang orang kurang sadar tentang Bahaya merokok sehingga mereka merokok dengan enak dan santai kapanpun mereka sempat. Sebenarnya banyak cara berhenti merokok, akan tetapi jika didalam hati sang perokok tersebut belum benar-benar niat untuk berhenti,pasti akan kembali merokok lagi.

Bahaya Rokok bagi kesehatan

               Menurut penelitian seseorang yang menghisap rokok setiap hari dapat meningkatkan resiko terkena kanker laring, paru-paru, kerongkongan, rongga mulut, gangguan pembuluh darah, gangguan kehamilan dan sakit jantung. Menurut riset seseorang yang secara rutin merokok 3 hingga 4 batang sehari, delapan kali lebih beresiko terkena kanker mulut jika dibandingkan orang yang tidak merokok. Bahkan hasil terbaru menunjukkan bahwa dalam perkembangannya merokok akan mengakibatkan kanker pankreas.

            Setiap tahun frekuensi penderita penyakit kronis akibat rokok semakin meningkat. Meskipun banyak riset dan bukti otentik bahwa merokok ibarat bom waktu yang bisa merusak kesehatan. Ini dikarenakan rokok memunculkan rasa kecanduan. Di dalam rokok terkandung sebuah zat yang bernama nikotin. Zat ini bisa menimbulkan efek santai dan inilah yang membuat kebiasaan merokok sulit untuk ditinggalkan.

• Asap Rokok mengandung 40 bahan kimia penyebab kanker, dan penyakit lainnya.
• Ketika merokok,beberapa bahan kimia akan menjelajah ke organ vital tubuh .
• Asap rokok juga mengandung Karbon Monoksida yang jika dihirup akan menggantikan fungsi Oksigen di sel – sel darah dan mengambil zat makanan dari jantung, otak, dan organ tubuh lain.
• Di dalam rokok terdapat Nikotin. Nikotin ngerangsang zat kimia di otak yang mengakibatkan kecanduan. Zat kimia ini merangsang kelenjar adrenalin untuk memproduksi hormon yang mengganggu jantung akibat tekanan darah dan denyut jantung meningkat.

Cara berhenti merokok

            Banyak cara agar kita bisa berhenti merokok. Tahapan pertama dan terpenting adalah niat yang besar serta sungguh-sungguh ingin berhenti merokok. Tanpa niat yang besar mustahil seseorang bisa berhenti merokok. Banyak mengaku diri perokok berat dan mengakui bahwa tekad mereka sangat besar untuk berhenti merokok. Namun ketika mereka keluar dan berkumpul kembali dengan teman-temannya yang merokok, keinginan itu muncul kembali. Inilah kenapa niat dan tekad itu yang paling penting ketika Anda ingin benar-benar berhenti merokok.
